RealGUIDE download: modules, pricing and system requirements
RealGUIDE is software for implant planning on CT (CBCT), surgical guide design and prosthetic design. It is developed by 3DIEMME, an Italian company based in the province of Como and a subsidiary of ZimVie. Both companies sell it: 3DIEMME through its own website and dealers, ZimVie through realguide.com. In the EU RealGUIDE is registered as a class IIa medical device, and in the US it has FDA 510(k) clearance K173041.
You can download RealGUIDE for free from official sources. What the free part includes depends on the product line: ZimVie's free PLAN already lets you place implants, while 3DIEMME's free START covers data import and diagnosis only, with implant planning in the paid PRO module. Guide design and prosthetic design are paid in both.
The current version is 6.0. It also runs in a browser, and AI prepares drafts: it suggests implant positions and builds a guide with the sleeves in place. Where a source document is dated, the date is given next to the figure.
Where to download RealGUIDE officially
Windows and macOS installers are on two websites: realguide.com/en/download (ZimVie version) and 3diemme.it/en/download-software-realguide (3DIEMME version). The 3DIEMME page also lets you try the web version. If you bought the license from a distributor, the distributor provides the installer and support; their contact details are in the software under Settings > Info. There is no reason to look for torrent builds: the manufacturer gives you the free module and the trial period itself.
Installation is standard: run setup.exe and follow the wizard. On first launch the software works offline and asks you to register. Click CREATE NEW USER, fill in the form and confirm your address via the link in the email. Then sign in with your email and password using LOGIN; check SAVE PASSWORD so you don't have to type it at every launch. One account works on as many devices as you like. The license is cloud-based, so no hardware key (dongle) is needed.
RealGUIDE modules: what is free and what needs a subscription
ZimVie's line-up for version 6.0 has five modules:
- PLAN — free with no time limit: superimposing a scan (STL) on the CT (DICOM), tracing anatomy, placing implants and abutments, sharing files with colleagues;
- GUIDE — designing surgical guides supported by bone, teeth or soft tissue and exporting STL for 3D printing or milling;
- WEB APP — working in a browser with no installation, new in 6.0. In the 5.4 brochure this slot was the APP module and compatibility read “Mac, PC, or iOS”; the 6.0 brochure says “Mac, PC or Web”;
- CAD — prosthetic design: crowns, bridges, bars, custom abutments;
- FULL SUITE — PLAN, GUIDE and CAD together.
Subscribers get SANDBOX free of charge, a set of free-form modeling tools: Boolean operations, splitting an object into parts with connectors, text on a surface. Cloud limits in the 2022 ZimVie brochure: up to 5 patients in the free PLAN and up to 100 in GUIDE.
3DIEMME has six modules: free START for data import and diagnosis, APP, PRO for implant planning, DESIGN for planning with guide export, CAD for prosthetic design and CAD+ with everything.
The trial period is described differently across documents. ZimVie's 2022 brochure offers GUIDE free for 30 days, and its Encode converter manual offers Full Suite for 30 days. 3DIEMME unlocks all PRO features for 30 days after installation and enables other modules for a trial on request. Note that during the trial you can download only two implant libraries.
How much does RealGUIDE cost? GUIDE, CAD and Full Suite prices
For current prices ZimVie points to realguide.com; official price lists from previous years give a reference point. ZimVie's US price list for 2022, annual commitment paid monthly:
- PLAN — $0;
- GUIDE Digital Doc — $49 per month, 1 click per month included;
- GUIDE Enterprise — $149 per month, 5 clicks per month;
- APP — $9 per month;
- CAD — $249 per month;
- Full Suite — $349 per month.
A 3-month package without the annual commitment costs more: GUIDE Digital Doc $75 per month, Enterprise $225, CAD $375, Full Suite $525. Extra clicks are sold in packs. For Enterprise and Full Suite one click costs $30, and in a pack of 500 it comes to $12 each. For Digital Doc it ranges from $50 for a single click to $20 in a pack of 500. ZimVie's Italian price list for 2023 has GUIDE at €49 and €149 per month, CAD at €190 and Full Suite at €290 (in the US in 2022: $249 and $349).
The 3DIEMME price list (undated, prices excluding 22% Italian VAT) offers both a subscription and a perpetual license. A 12-month subscription costs €90 per month for PRO, €190 each for DESIGN and CAD, €290 for CAD+ and €9 for APP. A perpetual license costs €2,900 for PRO, €4,900 each for DESIGN and CAD and €6,900 for CAD+, plus optional annual maintenance with updates. If maintenance on a perpetual DESIGN or CAD+ license is not renewed, the software keeps working, but surgical guide STL export becomes pay-per-click at €50 per case. With a DESIGN subscription STL export is unlimited.
ZimVie's price list does not explain what a click is. It is probably a guide export too, but this is not stated directly. The 2022–2023 prices show the order of magnitude, not today's cost: before buying, ask for a quote on realguide.com or from the distributor in your country.
RealGUIDE 6.0 system requirements
Requirements for 6.0 have gone up noticeably. According to 3DIEMME's requirements sheet of March 2026, on Windows you need:
- Windows 10 or 11;
- an Intel i5 or Ryzen 5 processor;
- at least 16 GB of RAM;
- at least 300 GB of disk space, which holds the patient database;
- a graphics card with at least 4 GB of vRAM; for AI features 3DIEMME recommends NVIDIA GTX or Quadro with compute capability 5.0 or higher;
- a 1920×1080 screen and a stable internet connection.
On a Mac you need macOS 13 Ventura, 14 Sonoma or 15 Sequoia and an M2 processor; the listed models are the 16" MacBook Pro and the iMac. The web version needs Google Chrome and a stable internet connection.
For comparison, the 2023 user guide for versions 5.1–5.3 listed Intel i5 or i7, 4 GB of RAM (8 GB for large datasets), a graphics card with 3 GB of vRAM, macOS 12–13 and iPad Pro with iOS 16. Two requirements from the same guide are still worth following: connect the computer to the network by cable (LAN), because data transfer over Wi-Fi can be slow, and add RealGUIDE processes to your antivirus exclusions while opening RealGUIDE's ports in the firewall.
RealGUIDE versions: 5.4, 6.0 and older projects
ZimVie released version 5.4 on July 31, 2024. It added one-click nerve detection, automatic bone and tooth segmentation, cloud prosthetic libraries and, for prosthetics, the Split Bar and Copy Denture workflows. Version 6.0 (ZimVie brochure, November 2025) added a web platform, AI teeth and implant placement, an AI guide draft and the SafeCut plugin for bone harvesting guides.
Before installing a new version, 3DIEMME recommends uninstalling the old one completely. Patients and projects are not lost: they are stored in the RealGUIDE50-DB folder. Do not delete that folder, or local data will be gone for good. Only the settings folder is cleared: RealGUIDE50, or RealGUIDE_3DIEMME in version 6. You can open it from the software via Settings → Show settings folder. Your custom sleeves and analogs are stored there as well, so copy them somewhere else before uninstalling.
Backward compatibility is limited: the current software does not open projects from versions before 5.1, such as 4.2. Partner builds of RealGUIDE use a different settings folder name, and their installer comes from the distributor.
Implant libraries for RealGUIDE
RealGUIDE downloads an implant library from the cloud right in the implant selection window, and in 6.0 the implant library updates automatically. Since version 5.4 prosthetic libraries are also in the cloud, so you do not have to install them by hand.
The DentalLibs catalog has 187 RealGUIDE library archives, almost always one per manufacturer: from Osstem and Straumann to the library of 3DIEMME itself.
RealGUIDE FAQ
Can I download RealGUIDE for free?
Yes, from realguide.com or 3diemme.it. Both product lines have a free module: PLAN from ZimVie (with implant placement) and START from 3DIEMME (import and diagnosis). The other modules are paid and can be tried for 30 days.
Does RealGUIDE run on a Mac?
Yes. Version 6.0 needs macOS 13–15 and an M2 processor. Without installing anything you can work in the web version in Google Chrome.
Why is the 6.0 update not showing up?
6.0 may not be out in your country yet: it is available only where it has been certified. 3DIEMME says it will send a notification once it becomes available.
Will projects from RealGUIDE 4.x open?
No. According to the user guide, the current software does not support projects from versions before 5.1.
